Dental Wellness Membership Plan
Here at North Orange Family
Dentistry our goal is to provide the
highest quality comprehensive
dental care at an affordable price. To
that end, we have created the North
Orange Family Dentistry Wellness
Membership Plan. This plan is not
insurance. It is a membership plan
offering discounted services, with no
deductibles and no yearly
maximums.

Dentistry Wellness
Membership Plan Benefits:
- 2 preventative visits per twelve (12)month membership period are included with the Annual Premium for each individual included in the plan.
Such preventative visits include:
- A basic teeth cleaning
- A doctor’s exam
- A fluoride application
- 1 set of necessary x-rays, as determined by the doctor
- 25% discount on in-office dental procedures
It is estimated that the costs for the
above listed services without
insurance and without membership
benefits is approximately $412.
Wellness Plan members also
receive 25% off on ALL in-office
dental procedures.
These procedures include:
- Sealants
- Fillings
- Crowns
- Extractions
- Teeth Whitening
- Bridges
- Dentures
- Partials
- Veneers
- Invisalign
- Clear Correct
- and many more...
Dental Wellness Membership Plan
Guidelines
- Family members eligible for the membership plan include spouse, and dependent children throgh the age of 23
- All membership plan fees are due and payable in full at the time of registration
- All Membership plan fees are NON-REFUNDABLE (even if services are not recevied by members)
- Any additional charges for services are due at the time of service
- The membership plan duration is for a twelve (12)month period, starting on the registration date
- Membership plan coverage automatically teminates at the end of the twelve (12) month period, unless renewed by the member(s)
- No refunds of premiums will be issued at any time, including if a member decides not to utilize the membership plan
- If the membership plan is utilized for orthodontic care, the plan member receiving the treatment must be a member of the membership plan for the complete duration of orthodontic treatment for the discount benefits to apply
Exclusions and Limitations
- This membership plan is not to be considered insurance., and is only offered to those without dental insurance.
- Members cannot use any other dental coverage with this plan, and this plan cannot be used to supplement dental insurance.
- This membership plan will not cover any portion of services provided by a specialist.
- The membership plan will not apply to costs of dental care for injuries covered under workers compensation
- The membership plan will not apply to dental care for which, in the sole opinion of the treating dentist or doctor, lies outside the realm of his/her capability
- The membership plan will not apply to hospitalization or hospital charges of any kind
- The membership plan will not apply to costs of dental care which is covered under automobile or medical insurance
- Covered benefifits of the membership plan are only available at North Orange Family Dentistry (7420 Gooding Blvd., Suite 200, Delaware, Ohio 43015), and referral made to an outside dentist will not honor the plan benefits
- North Orange Family Dentistry and its individual dentists reserve the right, in their sole discretion, to refuse to provide treatment to any individual plan member or dismiss any plan member as a patient
ANNUAL PREMIUMS:
- $299 for Individual Membership
- $250 for Each Additional Adult (Spouse or dependent children age 18-23)
- $200 for Each Additional Minor Child (Children up to age 17)
If the dental savings plan is paid for using
Care Credit, there will be an additional 15%
administration fee applied.
